Alternative procedures to surgical resection include minor interventions such as cryosurgery with liquid nitrogen, carbon dioxide laser therapy, and thermotherapy, all of which have been used with various degrees of success (Tagami et al, 1984; Kultner and Siegle, 1986; Pimentel et al, 1989; Hiruma et al, 1993; Bonifaz et al, 1997). For smaller lesions, cryotherapy is effective for lesions not in flexion areas, and may be beneficial in larger lesions as an adjunct to chemotherapy (Lubritz and Spence, 1978; Borelli, 1987; Kullavanijaya and Rojanavanich, 1995; Bonifaz et al, 1997; Bonifaz et al, 2001). The most commonly employed drugs are amphotericin B, flucytosine, and itraconazole, although there is experience with numerous antimicrobial agents (Table 26­2). Intravenous amphotericin B as single therapy appears to be minimally effective, and is often associated with adverse events after prolonged use (Bopp, 1976; Astorga et al, 1981; Fader and McGinnis, 1988; Restrepo, 1994; Rios-Fabra et al, 1994). Amphotericin B is used most commonly in combination with flucytosine for moderate or severe chromoblasomycosis (Bopp, 1976; Astorga, 1981; Fader and McGinnis, 1988; Restrepo, 1994).
